Coordinates patient referrals and provides assistance in case management negotiations. Receives and processes patient referrals from patients, physicians, and hospitals. Typically requires an associates degree with license (as a RN, LPN, RRT or CRTT) and two years of clinical experience. Enters and maintains case management data in system. Enters referrals into a computerized database and forwards facsimiles to appropriate health providers.
