Gets specific information from patient and enters information onto prescribed forms; information may include, but is not limited to, name, address, age, medical history, symptoms, present medications, previous hospitalizations, religion, person to notify in case of emergency, attending physician, insurance coverage, and party responsible for payment. Places completed records in order and files in patient's file. Schedules appointments for examinations according to nature of illness.
