Job Description:

Utilizes nursing processes and American Diabetes Association guidelines to initiate and promote ongoing diabetes management for diabetes patients. Requires a bachelor's degree in nursing or health education and three years of health education experience as well as certification as a Certified Diabetic Educator. Initiates and promotes ongoing diabetes management for non-insulin and insulin-dependent diabetes mellitus patients. Collects comprehensive patient health data and critically analyzes assessment data in determining or confirming the diagnosis or problem list. Identifies expected clinical outcomes from assessment data and diagnosis or problem list and individualizes expected outcomes with the patient and with the health care team. Develops a comprehensive plan of care that includes interventions and treatments to attain expected outcomes. Initiates or implements interventions and treatments for the plan of care. Provides comprehensive clinical coordination of care and case management. Implements strategies, interventions, and teaching to promote, maintain, and improve health among individuals, families, groups, and communities who have or are at risk for diabetes. Provides in-service training and education to ensure competence of staff providing diabetes management. Develops, organizes, and distributes diabetes educational resources and materials.
