Job Description:

Provides clinical pharmacy services support for formulary development and maintenance, pull-through development, and drug information services for a medical clinic or hospital. Writes, coordinates, and presents evaluations of drugs. Advises Physicians and reviews prescriptions issued by Physicians, or other authorized prescriber, to assure accuracy and determine formulas and ingredients needed. Provides clinical support for updating the formulary's clinical guideline sections. Creates formulary groupings and new charts and graphs for the clinical and drug listing sections. Prepares, reviews, and revises Physician reference sheets. Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs. Performs literature searches and analyzes data to answer questions surrounding drug therapy. Provides clinical material for newsletter, including formulary notification items. Presents and coordinates educational programs for the medical staff. Ensures that pharmacotherapy treatment recommendations in the disease management program are consistent with formulary initiatives.
