Coordinates various aspects of resident care to include admissions, discharge, resident assessments, and care plans with residents, resident’s families, staff and administration in a long-term care or nursing home facility. This position typically requires a Registered Nurse with long-term care experience or a Master of Social Work with three years of social work experience in a long-term or medical care facility.
Coordinates various aspects of resident care to include admissions, discharge, resident assessments, and care plans with residents, resident’s families, staff and administration in a long-term care or nursing home facility. for admission and medical equipment. Completes admission, pre-admission and transfer paper work. Collects pertinent documentation and communicates addition of new admissions to nursing and therapy departments. Plans, develops, organizes, implements and evaluates the admission process. Ensures that all admission procedures are followed in accordance with established policies. Develops and implements marketing strategies. Reports all concerns and complaints made by a resident or their family to the appropriate person. Conducts thorough assessments of applicants from phone inquiries and onsite assessments of applicants, as appropriate. Provides specific information regarding the admission process or status to families, consumers, community hospitals, and community agencies. Reviews application forms and completes pre-admission assessment forms to determine appropriateness for admission. Contacts insurance representatives to confirm authorization. Coordinates hospital returns. Maintains liaison with insurance companies, health maintenance organizations, managed care contract companies and other third parties to obtain insurance verification data and pre-certification approval for admissions. Responsible for records of admission, transfer, discharges, patient's necessary personal and financial data, and occupancy-census data.
