Community Mental Health Worker:
Provides strength and community-based rehabilitative mental health services, as described in the clients' case plans. Collects information about clients through interviews, observation, and tests. Develops and implements appropriate individual treatment plans for each client, based on clinical experience and knowledge, in order to meet his/her mental health needs. Provides individual and/or group mental rehabilitation services for clients.
Guides clients in the development of skills and strategies for dealing with their problems, and assists them in overcoming dependencies, adjusting to life, and making changes. Encourages clients to express their feelings, discusses what is happening in their lives, and helps them develop insight into themselves and their relationships. Helps individuals deal with thoughts of suicide; addictions and substance abuse; family, parenting, and marital problems; stress management; problems with self-esteem; and issues associated with aging and mental and emotional health. Provides and teaches self-advocacy and independence for families. Assists clients and families to engage in appropriate problem solving. Assists and advocates for clients in working with other agencies and professionals to ensure continuity of care. Assists clients in improving their social and interpersonal skills and knowledge of what is socially acceptable behavior. Acts as an appropriate role model for program clients by demonstrating socially acceptable and age appropriate behaviors. Provides crisis intervention as needed. Works in community with social service agencies, schools, and other juvenile facilities to advocate on behalf of the clients. Understands the public mental health system regulations and guidelines. Transports and supervises clients for program activities, classes, medical appointments, court appearances, and emergencies, or for other reasons as assigned.
