Clinical Pharmacist:
Responsible for providing clinical pharmacy services support for formulary development and maintenance, pull-through development and drug information services.
This position requires completion of Pharmacy D. degree or BS degree in pharmacy and clinical training and experience. Writes, coordinates and presents evaluations of drugs. Reviews prescriptions issued by physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed. Provides clinical support for updating the formulary’s clinical guideline sections. Creates formulary groupings and new charts and graphs for the clinical and drug listing sections. Prepares, reviews and revises physician reference sheets. Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs. Performs literature searches and analyzes data to answer questions surrounding drug therapy. Provides clinical material for newsletter, including formulary notification items. Presents and coordinates educational programs. Ensures that pharmacotherapy treatment recommendations in the disease management program are consistent with formulary initiatives. May instruct interns and other medical personnel on matters pertaining to pharmacy, or teach in college of pharmacy. (Source: eDOT Job Description) [-] Less
